This has a concurrent state, but has state transitions triggered from UI buttons.

I am not sure what you mean with point one - are your referring to generating a Document?
With point 2 You can set up Hyperlinks to Elements from within the document - Highlight a word - Context menu | Create | Link to Existing Element. But in reverse as there is no direct reference inside the Document that an Element can refer to - so this is not supported.

If the Discussion option is close to what you require then maybe can look at using the Team Review feature.
Both of these are accessible in the Free EA Lite Edition - so long the users have access to your repository (i.e. via cloud).
In the simplest usage the Team Review can be a document inside EA covering your points, but it can also cover a lot more.

A means of restricting the heading level is not something that is supported in the library we use. A work around that you can consider is to create a document in Word just with a TOC that is set to provide the first 4 headings, then import that document into a newly created Template in EA (context menu: File | Import).

As an alternative to creating Tagged Values - Connectors do have an Alias, this can be shown on the diagram by selecting the Diagram Property
 [ x ] Use Alias If Available.
The Shortfall is that you will see all the Element Aliases if these are set as well.

If the packages containing these elements are not reported in the Package tree that you are reporting then they won't be selectable in the output.

An extra option to Phil's comment is to simply drag the document from the windows file explorer onto a diagram. Then you have the option to save it as a reference or store it internally.

If you are wanting to reverse transform to a logical Class model, with atributes inside the Class rather than the ERD format that includes these externaly, then look at the transform scripts on:

With your Windows | Control Panel | Display options - do you have this set to another Size for Items - not 100%.
If so, try setting it back to 100% and check for any difference.

With the source code generation you do need  set the default Code Language using the Code generation Toolbar:

Also ensure that the Class element has the Language set correctly to c.

When set to C or C++ this should generate to c or cpp plus a .h file.
Once generated use the F12 key to view these.

With the Reporting templates for the Embedded Elements section - it is best to set this on, but leave it blank. 
Then it will replicate the sections under Package | Element - including Attributes etc.
These Package | Element | Attribute & Method sections need to be polulated with any necessary deatils in the template.

I am not sure if this is of use, but you can set an Action Element as a direct Hyperlink: Action | Other | Hyperlink - see the help page:

Once you set the URL - Double Clicking will open the page.
