Title: How do I create documentation for audit history?

I have an issue that I am not finding a way to document audit history for test cases.  The audit trail displays on screen but when trying to either create a new template or use the 'basic + audit' prefab template using the audit history and audit item boxes, I am unable to return any data.  I know this information exists and therefore should be displaying on my documentation.  Long story short, how do I create documentation for audit history within the test case module?
