Icons should be 16 by 16 bitmaps.
I've done that many times and it always worked just fine.

You might have the wrong size, or the wrong file format.
Make sure to set your background color to the magic grey: RGB 192.192.192

I've faced a similar challenge with icon sizes before, and I found that using a combination of CSS and SVG icons helped me achieve consistent sizing across browsers and toolboxes. You can set the width and height of your icons in CSS to ensure they appear uniformly.

That feels overly complicated. I've never had issues with 16x16 bitmaps.
Don't use .png or .jpg or similar because that doesn't work.
