Title: State Machine transitions with pre-conditions in model report template

Dear all,
I've created a state machine diagram with transitions. Transitions are linked with a pre-condition constraint.
My dream is to report this diagram into Word document that will show transition from one state to another state with all conditions related with this transition. I am struggling to create template sections that will give this view.
Do you have any working template that will cover this?

I would probably make an SQL fragment for that part.
