How to record Risk?

There are a few entries in the Help file about Risk, and I can find the Project, Metric Types, Risk menu option, but I can't see how to record a Risk against a particular item.

The help file shows (under 'Effort and Risk Management') a form that look like the thing I'm after, but I can't find how to get to it!!!!

Right-click on an object in a diagram, go to the "project management" menu item in the context menu, and then choose "Project Information".
