Allow named HTML output files

The current HTML output (where the .htm files and folders are numbered sequentially) has a degree of utility.

However, its usefulness would be considerably increased if the output files and folders were named as per the EA entries.

Name clashes (within the same folder) would have to be handled (presumably by _n for the subsequent ones n>=2)

However this would allow a mechanism for targetting the HTML output directly from other web enabled servers.

Naturally, maintaining the links would be an external activity and NOT the responsibility of EA.

Before I formally hit Sparx with a feature request...
