Defect and Change Reporting

After I have input changes and defects on various objects, packages, views,etc. how can I create a report that shows all changes and defects?  The only ones I can see are on individual objects-- views and package defects/changes don't seem to appear anywhere.  Also, is it possible to produce a report which ONLY shows changes and defects instead of having to know which package/view/object has defects and changes in the first place?

Your suggestion will be useful for me as well.
Hmmm...on 2nd thought, maybe EA expect us to record defects and changes, assign owners in the Todo first, then the owner will take action from there.
The owner will then record the defects and changes in the appropriate elements. The owner will also update the Todo item with the status, the elements/packages affected. So, u can get the location of the defects and changes from the todo list.
Is that a workable procedure? Bear in mind you could be working in a team.
I haven't found out how to print out the Todo report yet, though.
