RTF documentation - why does it show old diagrams?

Sometimes (not always) when I am actively modeling and creating RTF documentation, I see older versions of my diagrams showing up in the documentation! I mean, I changed the diagram in the last ten minutes, regenerated the RTF documentation, and the changes just do not show up in the documentation!

Where is EA picking up the older diagram images from and why? How do I stop this from happenning?

Probably you didn't save the diagrams. As opposed to model data, diagram data don't go to the eap file (or the databae) immidiately. And when you're modelling away with a lot of diagrams it's easy to overlook that some of them are not saved.

The diagram tabs at the bottom show an asterisk then, and they have a "save all" context menu.
