Having trouble with lost files empty folders

Hej,

We are using EA 7.1 together with an oracle db. this is connected to version control through Perforce.

The problem we have been seeing is that files sometimes get lost or foldes are emptied.

Have anyone else seen this problem.
Urgent tip/fix is needed
