Missing ToolBar/ Icon

I have just reinstalled EA and there appears to be a tool or icon missing.  There was an icon at the top which allowed me to place an object on a diagram to show the name, author, version, date etc.
The icon has disappeared.  
How can I get it back ?  I have tried the tool bars and cannot find it...
  :(

Select the Tools | Customize menu option, and on the Customize dialog click on the Toolbars tab. You can either select the checkbox for each appropriate Toolbar, and click on Reset, or you can just click on Reset All.

Does that bring your icon back?

I assume you are referring to a Diagram Note?  This can be accessed from the Common Toolbox, or from the UML Element Toolbar.

Also see RoyC's previous instructions which may help restore your toolbars if the icons have become mixed-up or the toolbars have been inadvertently modified.
