Why don't you start of with the example model provided by Sparx, and tell us where you runt into trouble.
I'm guessing that you will get better answers to precise questions then to expect someone to give you a full example.

As suggested by Geert, try looking at the EAExample.eap model provided with Enterprise Architect.  This model contains examples of Use Cases, Requirements and Class models, with relationships defined between them that can be traced (E.g. Use Case realizes Requirement, Class realizes Use Case).  The actual process of designing your project (writing use cases, etc) may be better addressed by gaining a better understanding of how to use UML in general.

For an example of behavioral code generation from a sequence diagram, browse to the Account class under Project Models > System Model > Implementation Model (PSM) > Java Model With Behaviors in the EAExample model.
