Add 'Assumption' in Requirement modelling

In requirement modeling, there is "Requirement", "Risk", "Issue" element that can be used. I would like to create custom element called "Assumption" for our requirement modeling, and with specific icon/diagram if possible. Also, if it is possible i would like this element to be present in "Requirement" toolbox

Can someone point me to the right direction on how to do this? I have been reading up about "Extending UML Profile", but I am not sure if this is the thing that I am looking for.
