v16 - Element Usage dialog needs indication of the Element involved

As noted in https://sparxsystems.com/forums/smf/index.php/topic,47300.msg275863.html#msg275863, Element Usage Diagrams can pop up arbitrarily.  They may contain tens of diagrams!  Trying to identify the element involved can be very problematic!  The element involved's name (or path) needs to be added to the Element Usage dialog, so we can unambiguously determine which element usage is being displayed!
