TFS work item integration inside EA

Hi all,

First time posting here, if it should be in a different forum, or there's a better channel for me to approach let me know - thanks for your patience.

I've been attempting to set up EA with MDG integration and have been successful for the most part. I've followed the steps listed here:

However, I've been trying to recreate the functionality displayed in this video:

I have yet to be able to get the Add-in -> Visual Studio -> Team Foundation Server to appear. I have a list of View Project Connections,  MDG options, import build scripts, help, and about. The project is, to the best of my knowledge, fully connected to a TFS project, including source controlled.

I am running EA 10.0.1009 corporate edition, with VS 2010, and MDG 4.5.1.
