Dave,
The tool bar varies on the diagram type. I had some problems reconciling the use of the 'Custom' area as well. In the end I just restructured the whole thing to reflect my current work method of Requirements-Uses Cases-Interaction Diagrams and then Class/Database  Diagrams.
I wasn't used to doing analysis diagrams so I have a folder just for them under Use Case view.

My current folder structure is now:

Application Specification
   Formal Requirements
   Non Functional Requirements
Use Case View
  Analysis Model
  Top level Use Case
Dynamic
Logical etc.

and it's really easy to take the default and create a structure to represent your development process

We have "Paste Appearence" button. Where is the "Copy Appearance" button?.

Well you could do worse than starting with the whitepaper http://www.sparxsystems.com.au/WhitePapers/The_Use_Case_Model.pdf

Personally I tend to realise Use Cases rather rapidly as Interaction diagrams and the constraints appear in the classes, and often as database rules. Just a note on a collaboration diagram can be good for this as well.

Find your audience and see what they are happy with  :)
