I have always filled in everything, including a "0" (zero) for service pack. The submit button was enabled.

This has become a habit, so I do it with the new form as well, regardless of whether the service pack box is marked as mandatory. So I don't really know if this is the issue, but it is worth a try.

If filling the service pack in turns on the submit button, I suggest you fire off your current bug report, then send another one that mentions the mandatory field issue itself.

I put a service pack # in.  No difference.  I just didn't have it in that screen shot.

Don't know. As I said, I've had no problems with the new form.

FWIW, I've only used the Registered Users version, and I have only used it with Firefox 2.0.0.13 (possibly 2.0.0.12 as well, but I don't think so).

Looking at your screenshot the email address line is highlighted.  For some reason the form thinks it is an invalid email address.  Please still send an email to sparx support about this as I can't do anything to help.

As Simon mentioned, in your screenshot it shows that the Email Address field is not being accepted (it is highlighted in red). This field not only checked to make sure there was content, but also tested the value to see if it was in the expected format.  We have removed the format checking on all bug report / support request pages for the Email Address field and it should now only check that there is some value.  Try using the bug report page again and see if there is any improvement.  Any further problems, please send an email to support.
