Importing from CSV files

I am trying to create "stubs" for future sequence diagrams as placeholders in a model from a CSV file containing "Name" of the sd, and "Notes" of the sd.

I've tried everything that came to my simple mind and  that was offered as (poor) guidance from the help file. Could be that I'm thick, but I just can't figure it out.

Can anyone tell me:

what (aside from name,notes) other fields my CSV file has to contain to assure an sd placeholder stub gets created
  • if no additional fields are required in my input CSV file, what else I have to do
