HTML Documentation has incomplete contents.htm

When I generate HTML documentation for a package with several subpackages, each with multiple elements, the HTML generator gives an incomplete contents.htm for some subpackages and not others. One subpackage, for example, has about 12 elements for which correct ea##.htm pages have been generated. But the contents.htm only contains a link to one of them. Is this a known bug?
