Maintenance - Changes, Issues

Hi,
I've been trying to work out the best way to record Changes and Issues. I like the way the Maintenance view has the items structured but can't seem to get a report from it in the format I require. I want to be able to produce a table that lists all the changes or all the issues. The Maintenance RTF breaks things into Elements so it is all scattered in separate tables.

Is there a way to alter this report to group the items in one table and exclude elements that don't have changes or issues?

I really need a change log and issue log that I can work from to produce reports for my manager as well as work through the items. I did look at the alternative of adding Change elements and Issue elements to the diagrams but my developers aren't keen on those diagrams becoming crowded with these.

Any suggestions?
