Fixed two bugs introduced in 1.0.18
 - Classes at root level caused error on merging.
 - Classes in sub-packages would re-appear in merge dialog after successful merge.
Both fixes affect org.sparx.mdg.eclipse_1.0.19.jar only - the EA Add-In DLL was unchanged from 1.0.18.

Encapsulated Plugin into a single jar file that the user needs to copy to the plug-in folder (org.sparx.mdg.eclipse_1.0.18.jar).
Add-In DLL version number updated to 1.0.18 to correspond with Eclipse Plug-in.
