Insert document artifact

Another RTF document generation question:

I want to include the content of a 'Document Artifact' element in the generated document before the 'Table of Contents'. Is there a way to specify in the rtf template to include the content of a specific element?

Also, what is the reason I cannot see an 'Insert Template' context menu while editing a template, for inserting template fragments?
