New mulit-level lists and list overrides

I'm trying to start a new numbering scheme for the appendix in a document. I want the numbering to restart and I want the header 1 level to include different text. I've created new styles and a new list override, and as far as I can see everything is identical to the standard header styles and list overrides, but it doesn't work. I'm having two problems:

• I sometimes get the extra text I want on the Appendix level 1 header, and sometimes not, I'm not sure which option is affecting this or if it's a bug. It has worked some times.
• When it iterates through a set of packages, everything comes out as level 1 rather than the list level numbering I'm after.
