MDG Integration for EA.

I am evaluating MDG Integration for EA (ver. 10, Build 1006) and Visual Studio 2012 (Premium ver. 11.0.61030.00 Update 4). It is difficult because the only Help materials I can find on the Sparx site refer to older versions of Visual Studio - e.g. http://www.sparxsystems.com/resources/demos/vsintegrate/vsintegrate.html.

In VS2012, (a) how do I open the EA Explorer pane *before* I open a Team Project or local .eap file, and (b) how do I attach a UML model to a project? (the 'Attach' option does not seem to be available from the VS2012 menu bar or context menu.)
